After submitting the Enrollment Application (#1) and upon acceptance, the forms listed below (also found in the back of your Handbook for Parents) are required to be submitted prior to the child’s first day of attendance:

 

  1. Enrollment Application (PDF)

  2. Discipline and Behavior Management Policy (PDF)

  3. Child's Care and Emergency Information Card

  4. Media and Photo Release

  5. Immunization history (updated copy)

  6. Child's Medical Report, Form 12/99 (preschool)

  7. Kindergarten Health Assessment Report, Form PPS-2K (blue)

  8. Copy of Certified Birth Certificate (Kindergarten and above)

  9.  Tuition Agreement (PDF)

10. Authorization for Release of School Records (PDF) (For students transferring from another school)
